Delirium and Dementia Outreach Team (Alexandra Centre)

The Dementia and Delirium Outreach Team provide vital support in Sunderland to those suffering from dementia and delirium, their families and carers. The team operate from the Alexandra Centre.


The Dementia and Delirium Outreach Team (DDOT) are part of the Care of the Elderly department.

The Trust's DDOT support patients with or at risk of cognitive difficulties such as Dementia and Delirium. The service aims to assess all older people admitted to the Trust with or at risk of cognitive problems. Referrals are only be made from within the Trust.

The service also has a role to support carers and staff within the trust by providing information, education and specialist input to the wards. People with dementia can be admitted to hospital due to many reasons such as infections, heart failure, falls or social issues. These patients have complex needs and require specialist input.

There is strong evidence that therapeutic activity and basic care improve outcomes for those experiencing dementia. The Alexandra centre aims to act as a centre of excellence to provide this care and raise standards across the trust by using an in reach / outreach service for cognitively frail in-patients. We also provide an outpatient follow up clinic for patients who have had delirium.
