Community Geriatrics
The Trust’s Community Geriatric team provide a vital service to Sunderland’s community, helping avoid unnecessary admissions into hospital for elderly residents and ensuring they are cared for compassionately and effectively. We work with existing community teams to optimize the care of Elderly patients in the community.
Community Geraitrics is part of the Care of the Elderly Department.
The Sunderland Community Geriatric service is a consultant led service that allows proactive management of frail older residents in Sunderland to help them stay at home and prevent acute admissions by:
- Performing comprehensive geriatric assessments
- Supporting use of advanced care planning
- Improving communication between primary and secondary care
- Provision of education and training to upskill the workforce supporting patients in the community

The department’s core hours are 08:30 to 16:30 Monday to Friday.
Community Geriatrics is based at Sunderland Royal hospital. Clinics are also available around the city to allow patients to be seen nearer to home. Weekly clinics are offered at the following locations:
Washington Primary Care Centre
Houghton Primary Care Centre
Monkwearmouth Hospital
Seaham Primary Care Centre
We can also see those patients who are unable to be brought safely to clinic in their own home.
