Freedom of Information Act

Freedom of Information Act 2000 allows anyone – no matter who or where they are – to find out whether information is held in the Trust and, if it is held, to have access to it subject to certain exemptions. Further information on the Act can be found via the links below.

If you are interested in information about City Hospitals Sunderland, please first take a look at our Publication Scheme (below) which describes the work of the Trust and outlines the material that we routinely publish.

If you want to request information that is not contained in the Publication Scheme, you need to make a General Rights of Access request in writing to:

Freedom of Information Officer
Department of Corporate Affairs
City Hospitals Sunderland NHS Foundation Trust
Sunderland Royal Hospital
Kayll Road
Sunderland   SR4 7TP

Fax: 0191 514 0220

Your request must give a contact name and address (if you are contacting us by email, an email address is sufficient). It will help us respond to your request quickly if you can be as specific as possible concerning the information that you are requesting.

We want to find the information for you promptly. To help us process your request efficiently and identify exactly what information you are requesting, we are providing a form that you may like to download, complete and return to us at the above address. You do not need to use this form to make a request.

Re-Use of Information

Any information we provide following your request under the Freedom of Information Act will not confer an automatic right for you to re-use that information, for example to publish it. If you wish to re-use the information that we provide and you do not specify this in your initial application for information then you must make a further request for its re-use as per the Re-Use of Public Sector Information Regulations 2005. This will not affect your initial information request.

Freedom of Information Response Satisfaction Questionnaire

If the Trust has recently responded to you following a FOI request, it would be helpful to assist us in maintaining and improving our service, if you could complete the attached form indicating the level of satisfaction with the services received.  Please return to
